Q: Should our AI documentation be digital or physical?
A: Both, ideally. Maintain a digital evidence drive that’s organized, searchable, and backed up. For accreditation site visits, also prepare physical binders with key documents—governance policy, tool inventory, representative DPAs, training records, and sample evidence. Many evaluators still prefer flipping through a well-organized binder, and having physical copies as backup ensures you’re prepared even if technology fails during the visit.
Q: How do we handle documentation for AI tools that faculty adopted without institutional approval?
A: When you discover unapproved AI tools in use, don’t panic—document. Add them to your inventory, assess their compliance status, and either bring them into your approved framework (if they meet standards) or migrate away from them (if they don’t). Document the discovery, the assessment, and the resolution. This demonstrates responsive governance, which is actually a positive sign for auditors. What they don’t want to see is ignorance—you claiming no AI tools are in use when a quick faculty survey would reveal otherwise.

Q: Do state authorizers ask about AI documentation?
A: A growing number do, particularly in states with active oversight agencies. California’s BPPE, the Texas Workforce Commission, and the New York State Education Department have all begun including technology governance questions in their review processes. Even in states that haven’t formalized AI-specific requirements, demonstrating strong AI governance in your authorization materials positions you as a well-run institution—which never hurts.

A: Keep a copy of each vendor’s VPAT (Voluntary Product Accessibility Template) in your compliance files. Conduct your own accessibility testing when possible. Document any accommodations you provide for students with disabilities who use AI tools. If your institution grows to 1,000+ students or deploys a large number of AI tools, a dedicated role may become justified.

A: While AI isn’t currently a formal element of Title IV program reviews, FERPA compliance is—and AI tools that process student data are part of your FERPA compliance surface. Maintain your FERPA audit trail, vendor DPAs, and data handling protocols so they’re accessible on short notice. Federal program reviews can be announced with limited lead time, so your documentation needs to be audit-ready at all times, not just when you know a visit is coming.
Q: What should we prioritize if we can only document a few things?
A: If you’re resource-constrained, prioritize in this order: (1) AI tool inventory with FERPA compliance status for each tool, (2) Data Processing Addendums from vendors handling student data, (3) AI governance policy, (4) Faculty training records, (5) Academic integrity code with AI provisions.
